Isoman Comorbid Covid-19 Patients, Pay Attention to Drugs Taken

In order to stop the transmission of Covid-19, one of the mandatory protocols for individuals who are infected or positive for Covid-19 without symptoms or mild symptoms is to undergo self-isolation. Patients with chronic diseases, such as hypertension and diabetes who are positive for Covid-19, are indeed advised to go to the hospital because of the high risk.

Described by dr. Jeffri Aloys Gunawan, Sp.PD, in Instagram Live Good Doctor and the Indonesian Shopping Center Retailers and Tenants Association (HIPPINDO) Saturday (28/8), self-isolation should be well prepared, especially for people with special conditions.

Covid-19 Horse Saddle Syndrome and Comorbid Chronic Disease

According to dr. Jeff has three phases of COVID-19 and horse saddle syndrome, namely the first phase, the pulmonary phase, and the cytokine storm phase.

“Treatment must be done aggressively before entering phase two or phase pulmonary, let alone to enter phase three or cytokine storm. Especially in people with comorbid chronic diseases such as diabetes or hypertension and the elderly, where the risk of death increases sixfold,” he explained.

Comorbid, added dr. Jeff, is one indication the patient should be hospitalized. Especially if there are more than two comorbid diseases, it is recommended to be treated like a moderately symptomatic COVID patient. “However, people with comorbidities can be isoman if their condition is controlled with routine medication,” explained dr. Jeff.

During isomanism, people with comorbidities are advised to continue to take their medication regularly and monitor oxygen saturation. The elderly usually take a lot of drugs or polypharmacy so that they need to be monitored by a doctor regarding which drugs should be continued, drugs that need to be stopped temporarily, and drugs whose dosages need to be adjusted or changed.

In diabetic patients, for example, blood sugar levels usually rise due to Covid-19, so they require insulin injections. When taking polypharmacy, do not hesitate to consult a doctor. If it’s difficult to go to the hospital, take advantage telemedicine or online consultation with a doctor.

Medicines and supplements that must be prepared

The supply of medication during an isoman is usually a concern for the patient. According to dr. Jeff, now there are lots of isoman packages available, packaged in boxes and equipped with a list of drugs and their dosages. “We have to be critical and know what it contains. Don’t just accept the package and take the medicine right away. There are a lot of drugs in the package, maybe dozens,” he explained.

Here’s a guide to taking medication during isomanism:

  • First, match the drug to the list. Check yourself, which category do we fall into: OTG, light, medium, or heavy? For mild symptoms the medicine is simpler, enough vitamins C, D, zinc, also vitamins B and E.

  • Additional medication—usually an antiviral (favipiravir), and possibly antibiotics. However, there must be indications, and are usually found by doctors / health workers. For antibiotics, the indication is secondary infection, usually by bacteria H. influenza or Streptococcus. These two bacteria often become opportunistic infections of Covid patients. If there are signs of this bacterial infection, especially if there are atypical/atypical signs, antibiotics may be given, usually azithromycin.

  • If there are symptoms of cough, phlegm is checked first with a lab swab examination, whether there are secondary infectious germs.

  • For severe symptoms, antivirals may also be required, and anticoagulants (blood thinners) may also be considered. This must be based on the doctor’s consideration, do not consume alone.

  • For severe symptoms, steroids are required. Usually if there is a decrease in oxygen saturation, so oxygen is needed. But there is a misunderstanding.
